British Red Cross offer short-term practical and emotional support to help people improve their well-being and quality of life and live independently in their community. The services are tailored to each person’s needs and goals.
Home from Hospital Services helps thousands of people following a short stay in hospital and prevents unnecessary hospital admissions by providing support at home.
What can the service do for me?
Some examples services include:
> Resettlement at Home.
> Time limited practical & Emotional Support/Confidence building.
> Reablement to access the community.
> Short term Shopping Support.
> Signposting / Supported referrals to community services.
How much will it cost me?
The service is free of charge. You will receive a visit either on the ward or at home with an option of up to 3 weeks support at home, with can offer a further 4 weeks of telephone support of one phone call a week if required.
How can I access this service?
Referrals are accepted from health and social care professionals, community hospitals, GP’S, Social Workers & many others.
What happens next?
Once your details are received, you will receive a phone call to arrange a day and time to visit.
